Since you've already got a rough edge that needs to be repaired, could
you open up one side of the upper block a little more and insert your
concrete from there?
In other words, build your form so that it covers one side and the
bottom of the block. Brace the bottom board from below. Then fill the
void from the side you opened up, using a stiff mix, until the void is
full. Then attach the last side of the form to hold it all in.
